Telehealth Informed Consent
Florida Stat. §456.47 and professional ethical guidelines require that we obtain your informed consent for telehealth. By checking the telehealth‑consent box during account creation or scheduling, you agree to the following:
3.1 Description of Telehealth
Telehealth involves the delivery of health‑care services via electronic communications (video, audio, text) allowing clinicians to diagnose, consult, treat, and educate at a distance. All major platforms we use (e.g., Zoom for Healthcare, SimplePractice) are HIPAA‑compliant.
3.2 Potential Risks
- Transmission of PHI could be interrupted by technical difficulties.
- Unauthorized access if security measures fail.
- Suboptimal clinician assessment of non‑verbal cues.
3.3 Benefits
- Increased access to care, particularly for geographically remote patients.
- Reduced travel time and associated costs.
- Continuity of care during public‑health emergencies.
3.4 Your Obligations
- Confirm your location at the start of each telehealth session so the clinician can direct emergency services if needed.
- Ensure a private, quiet setting with reliable internet.
- Do not record the session without mutual written consent.
3.5 Consent Withdrawal
You may withdraw your consent at any time by notifying your clinician in writing. Such withdrawal will not affect prior care already provided.
